Believe it or not, some researchers believe that some kinks/fetishes are largely innate to the way a person is wired, and that these preferences might've evolved because humans have historically lived in societies without modern-day levels of hygiene or "first-world" standards of sanitation. For instance, many people find it hard to get aroused when they're disgusted by a given sight or smell.... so people who COULD get aroused in the presence of urine/feces/vomit/unwashed genitalia/B.O., etc., or who could block out their sense of disgust and get aroused by focusing on a particular other body part or object instead, would've had a reproductive advantage.
In other words, kinks and fetishes seem to be human universals, rather than a "first world problem" — and might very well stem back to places and times when life was anything BUT "first world" in its conditions. Interesting to ponder. :)
